The Association of Family Health Teams of Ontario (AFHTO) works to support the implementation and growth of primary care teams by promoting best practices, sharing lessons learned, and advocating on behalf of all primary care teams.

AFTHO is leading the charge to create a stronger, more equitable healthcare system for Ontario and are on a mission to make primary care better for everyone in Ontario, both patients and providers. They’re a hub for collaboration in care, where evidence-based practice is shared and a community of innovators unites to transform the system.
They fight for fair, sustainable funding to ensure primary care teams have the resources needed to deliver top-tier care. Together with nearly 200 member organizations of almost 5,000 healthcare professionals, administrators and staff.
