Gravenhurst Health Hub delegation well received at recent AFTHO conference.
This right here is a picture of collaboration. Members of the Cottage Country Family Health team, local physicians, the Rotary Club of Gravenhurst, David Gravelle, physician recruiter for Muskoka Almaguin Ontario Health Team (MAOHT) and Town of Gravenhurst Mayor Heidi Lorenz, assembled at the Association Of Family Health Teams Of Ontario (AFTHO) conference to host a panel to show our community's model for solving the health care crisis that many Ontario cities are facing.
The team fielded questions in hopes that they might inspire other rural towns to move the needle forward in connecting more of their citizens to local primary health care reducing the need for them to go to emergency rooms in out-of-town hospitals for basic health care.
The Association of Family Health Teams of Ontario (AFHTO) works to support the implementation and growth of primary care teams by promoting best practices, sharing lessons learned, and advocating on behalf of all primary care teams. Evidence and experience shows that team-based comprehensive primary care is delivering better health and better value to patients. They are the advocates, network and resource for team-based primary health care in Ontario.
