A community in crisis; a town united.

Where there is community need, there are Rotarians. The Gravenhurst Health Hub project is the collaboration of government, citizens and service groups in the town of Gravenhurst, Muskoka, to resolve the long standing lack of available primary health care for the residents of this amazing town. Goals & Objectives

  • To provide access to urgent and after-hours assessment and care for all residents, with a focus on unattached patients.

    • Reduce pressure on hospital emergency department for low acuity conditions (CTAS 4 and 5 visits).

    • Provide urgent and episodic care to residents without a local primary care provider.

  • To expand access to specialists by creating new programs for visiting specialists through a collaboration with the University Health Network/University of Toronto and others.

    • Provide turnkey services to visiting specialists to see patients, with follow-up provided by a nurse practitioner.

    • Increase access and reduce wait times for needed specialist services.

    • Provide rural learning opportunities for medical residents.

  • To provide a full suite of diagnostic services to the residents of Gravenhurst and Muskoka.

    • Laboratory specimen collection services in partnership with Dynacare.

    • Xray and Ultrasound

    • Reduce travel for area residents for diagnostic services.

    • Reduce wait times for diagnostic services.

    • Provide convenient access to long-term care (LTC) and retirement homes.

  • To provide a common front office and shared office space with complementary health and social service providers.

    • Provide affordable, purpose built, space for co-location of  local health and social service providers.

    • Increase access to needed community mental health services through partnership with Canadian Mental Health Association (CMHA) and Family Connexions.

    • Facilitate collaborative initiatives by use of common meeting space and services.

    • In partnership with Paramed expand home care clinic for wounds, IV antibiotics etc.

    • Provide common meeting space for interprofessional team collaboration.

    • Provide local access and increased availability of community paramedics.

    • Expand access to palliative care advice through partnership with Hospice Muskoka.

  • Ensure all residents of Gravenhurst have a primary care provider supported by an interprofessional team, including nurses, pharmacists, mental health workers.

    • We are exploring opportunities to offer new programs and services to meet community needs, including:

      • Geriatric assessment

      • Women’s health clinic

Our PLAN

Our plan is ambitious due to the crisis in primary care access in our community.  To successfully meet this timeline we need support from all levels of government and from the community.

Phase 1: 2025

Establish strong community and government partnerships. Finalize the design. Secure conditional lease agreements with various health and social service tenants. Launch fundraising events. Develop comprehensive marketing strategies and community engagement plans to effectively reach and involve local residents. Complete basic infrastructure renovations and create space for new primary care providers. Recruit new primary care providers. Explore opportunities to create a Primary Care Teaching Clinic in partnership with the Northern Ontario Medical School.

Phase 2: 2026

Continue fundraising. Undertake leasehold improvements for new programs and services. Occupancy by new tenants and programs. Complete first floor renovations. Negotiation and secure diagnostic imaging (xray and ultasound partner). Renovations and clinic setup to accommodate new primary care services. In addition, we are planning if provincial support is confirmed to launch an expanded after-hours care clinic, along with a dedicated women’s health clinic, geriatric assessment, expanded laboratory services and pilot an innovative visiting specialist program.

Phase 3: 2027

Continue fundraising. Complete the diagnostic suite to enable onsite  diagnostic imaging services by increasing capacity and enhancing the variety of imaging options available, while also securing strategic partnerships with reputable diagnostic providers. Actively recruit additional healthcare professionals to strengthen the team and improve patient care. Continuously evaluate the hub’s impact on the community and explore further growth opportunities to meet the evolving healthcare needs of our patients.
